“I agree that photographs/images/video footage taken of me at during my employment under the AMPLIFY program may be used for publicity materials including appearing on the Community Sector Council NL website. I acknowledge that my participation is voluntary and no fee will be paid to me.”

PARTICIPANT CONSENT TO BE CONTACTED IN THE FUTURE:
By participating in AMPLIFY, you agree that the Community Sector Council Newfoundland and Labrador (CSCNL) may contact you in the future to learn about your employment, education, or overall experience since participating in AMPLIFY.

This information helps us evaluate and improve the program. We will only contact you using the information you’ve provided. Participation in follow-up is voluntary; you can choose not to respond.
