AMPLIFY: Terms and Conditions (Program Participation)

PRIVACY/CONFIDENTIALITY:
Personal information provided in the form will be collected and protected under applicable privacy laws. Information collected is used in maintaining your registration in this program, corresponding with you, and for statistical purposes for training in the province of Newfoundland and Labrador.

PHOTO/VIDEO DISCLAIMER:
Attendees at any of CSCNL’s events across Newfoundland and Labrador do hereby give permission for event management to use any photos and/or videos taken during the event that contain their likeness in any respectable manner without compensation. This includes, but is not limited to, the use of these photos and/or videos in print or electronic promotional material and/or on CSCNL’s website and/or in the organization’s social media channels.

YOUTH PARTICIPANT CONSENT TO RELEASE INFORMATION:
“I give my consent for Community Sector Council Newfoundland and Labrador Inc. to release the information contained in the Intake and SIN forms to the Government of Newfoundland and Labrador, the Department of Jobs, Growth, and Rural Development (JGRD). I acknowledge that the information disclosed to JGRD will be administered by JGRD in accordance with the Privacy Act (R.S.C., 1985, c. P-21) and the Department of Employment and Social Development Act (S.C. 2005, c. 34). I acknowledge that I have a right to obtain access to that information from Canada under the Privacy Act (R.S.C., 1985, c. P-21) and the Department of Employment and Social Development Act (S.C. 2005, c. 34).

This information will be used to confirm my eligibility to participate in the AMPLIFY program and to help determine appropriate services and supports during my participation in the project. This information will be provided to JGRD for the evaluation, research, and accountability of AMPLIFY. I may be contacted in the future by JGRD about my participation in the program.”

As funding for AMPLIFY is provided by JGRD, the department requires certain information regarding participants and results. Providing your personal information will never result in any decision made about you outside of this program. You have the right to file a complaint with the Privacy Commissioner of Canada regarding the Government institutions’ handling of your personal information here.

This information is used to:
• Measure the results of the AMPLIFY program and evaluate its success
• Evaluate more generally, the success of the AMPLIFY program in achieving its objectives
• Report annually on results and meet its obligation of accountability to the NL government and the Canadian public for the operation of the AMPLIFY program by reporting on the outcomes of the program and its success in achieving its objective.

YOUTH/HOST ORGANIZATION FEEDBACK:
“I give my consent for Community Sector Council Newfoundland and Labrador Inc. to release the information contained in any feedback form to the Government of Newfoundland and Labrador, the Department of Jobs, Growth, and Rural Development (JGRD). This information may be included in final reports submitted to the JGRD as well as within public documents or social media posts created to advertise the program or demonstrate CSCNL’s impact.”
